Endorectal Sonography in the Evaluation of Anal Canal Rectum and Anorectal Disease

Abstract
Knowledge of normal sonographic anatomy of the anorectum is essential in the evaluation of anorectal disease. We describe the anatomical status and the sonographic finding in variety of diseases in anorectum.
Endorectal sonography was performed in 27 patients for preoperative examination of anorectal disease and 24 normal patients for evaluation of anatomy of anorectal region.
At the anal canal, the internal anal sphincter was visualized as a linear hypoechoic band(thickness: 1.75¡¾2.13 mm, length: 22.38¡¾25.58 mm) which it was seen in continutiy with the muscularis layer of the rectal wall. The external anal sphincter
was
seen as a thicker echogenic band(thickness: 5.50¡¾6.00mm, length: 19.33¡¾27.46 mm) just outside the internal sphincter.
Endorectal sonograms show five layers in the rectal wall the first echogenic line corresponds to the interface between the water balloon and the mucosa. The innermost hypoechoic line represents the deep mucosa and muscularis mucosa. The third and
most
echogenic linde delineates only the muscularis propria. The most peripheral line, the fifth echogenic line corresponds to the interface between the muscularis propria and perirectal fat. Twenty-seven patients of disease group consisted of rectal
cancer(n=14), fistulous absceses(n=8), hemorrhoids(n=3), fissure(n=I), Future studies are expected to produce valuable benefits in the evaluation of anorectal diseases.
